To ensure your appointment proceeds smoothly, please arrive 15 minutes before your scheduled appointment time. You can download and print intake forms in advance or complete them upon arrival at our 60 Lafayette Street clinic.

Do I need insurance to visit the UB Clinics?
No! UB Clinics operates on an affordable, direct fee-for-service schedule with no insurance required. Our fees are significantly lower than typical private practice rates to ensure equitable healthcare access for the entire community.
Who will be providing my care during the visit?
Patient care is provided by advanced clinical student interns from UB’s graduate and doctoral health sciences programs who have completed rigorous pre-clinical requirements. Every treatment plan is directly examined and authorized by a licensed, board-certified faculty clinician.
Where are the clinics located and where do patients park?
The clinics are located at 60 Lafayette Street, Bridgeport, CT 06604, on the corner of Lafayette and University Avenue. Dedicated patient parking spaces are available right in front of the clinic entrance.
